Our cash price is $4000, which includes review of records prior to consultation, ultrasound if necessary, and surgeon fee for laparoscopic surgery, as well as follow up visits.

WHAT DO I NEED TO HAVE DONE BEFORE MY REVERSAL SURGERY?
Prior to evaluating your case, we will need the operative note from your sterilization surgery, and results from a hysterosalpingogram. We also recommend a thorough evaluation of ovarian function as well as evaluation of your partner's or donor's sperm function. These results can be faxed to our office or brought to your consultation. We can also arrange to have these documents sent to us or have appropriate studies ordered as needed. There may be additional charges for additional studies that may be required, which may be covered by insurance.
IS TUBAL REVERSAL COVERED BY INSURANCE?
Check with your insurer. Our surgical coordinator may also be able to review your benefits. In most cases tubal reversal is not covered by insurance, but if you have advanced reproductive benefits you may have coverage.
